Traditions Behavioral Health offers its full-time Employees health insurance coverage, paid for by Traditions Behavioral Health. The Employee’s family may be added to the health insurance at additional cost to the Employee. Traditions Behavioral Health offers the choice of three health insurance plans:
- United Healthcare PPO 20 (California Select-Plus)
- United Healthcare HSA (California Select-Plus)
This is a $20 co-pay plan.
Traditions Behavioral Health offers coverage through Delta Dental. There is a $50 Copay per year which is waived for diagnostic and preventive services. The calendar year maximum per patient is $1,000. There is no charge for diagnostic and preventive (x-rays, exams, consultations and cleanings). Basic services (restoration, oral surgery, endodontics, periodontics and sealants) are covered at 80% with crowns, jackets, cast restorations and prosthodontics (some limitations on prosthodontics) are covered at 50%. This plan does not offer Orthodontics.
Please note that Delta Dental does not offer “open enrollment” and will not allow eligible employees or dependents come onto the plan after initial enrollment. This is a one time offer only.
DISABILITY AND LIFE INSURANCE
Traditions Behavioral Health pays for Employee disability through Standard Insurance Company. The LTD benefit is 60% of the first $10,000 for your monthly pre-disability earnings reduced by deductible income. The maximum is $6,000 before reduction by deductible income and the minimum is $100.00. The benefit waiting period is 90 days. The maximum benefit period is determined by your age when the disability begins. The life insurance is two (2) times your salary to a maximum of $425,000.
Traditions Behavioral Health offers a 401k retirement plan administered by The Newport Group. The benefit is $0.50 on the $1.00 up to 6% of your gross providing you are contributing 6% of your gross. If you are only contributing 3% of your gross TBH will match $0.50 on the $1.00 to 3% of your gross while if you are contributing 15% of your gross TBH will match $0.50 on the $1.00 to 6% of your gross. Each employee has control over their investments through Verisight’s web site. There are about 15 plans to disburse contributions into ranging from conservative to aggressive. Vesting is 20% per year on employer contributions, employees are fully vested after 5 years of service.
401(k) participation begins the first quarter (Jan 1st, April 1st, July 1st or Oct 1st) after date of employment. (Example: Employment date isJanuary 1st, entry date to the 401(k) plan is April 1st)
Traditions Behavioral Health will reimburse full-time employees for their California Medical License and DEA renewals. Effective date May 1, 2016.
PTO AND CME
Traditions Behavioral Health offers its full time employees 5 days of CME time per year as well as accrued vacation days. The PTO and CME is on an accrual basis, accrued per pay period worked. Any un-used PTO at termination is paid out. Any un-used CME is not paid out at termination. Three months advance notice is requested for extended vacations for TBH to arrange coverage. If days off are required on shorter notice they will be permitted if you arrange coverage and if time off is available. All time off requests must be in writing and approved by the Medical Director and the facility in advance.
Traditions Behavioral Health’s accrued vacation days for full time employees are as follows:
1-3 Years of Service = 15 Vacation / Sick Days
4-10 Years of Service = 20 Vacation / Sick Days
11-15 Years of Service = 25 Vacation / Sick Days
16-20 Years of Service = 30 Vacation / Sick Days
21+ Years of Service = 35 Vacation / Sick Days
In addition there will be twelve (12) paid Holidays that are listed under Staff Resources.
